The practice of Japanese midwives toward pregnant-women in the maternity home ―Focus on what midwife touches―

Abstract
Purpose
The purpose of our survey is to reveal the practice of Japanese midwives toward pregnant women in the maternity home. And bringing out some evidence of midwifery judgment based on touching pregnant women.

Method
The questionnaire of the study was sent to the maternity homes (218 institutions), and the effective reply was used as data. The maternity home is the institution that the midwife opens independently. The survey items are the number of the attending midwives in the pregnant-women medical examination, medical examination time, the part which touches the body of a pregnant woman, the purpose of touching it, the time which touches it, etc. The ground total of the quantitative data was carried out and qualitative data was performed a content analysis.

Result
The numbers of reply institutions were 67 (30%). The average medical examination time spent on one pregnant woman was 48.1 minutes(n=62, SD=±17.2). The longest time which the average of the total of the time to touch the body of the pregnant women in one pregnant-woman medical examination was 21.9 minutes (n = 98, SD=±14.8). The part which touches was mainly the leg from the abdomen. The purpose was that plan communication and to promote relaxation as well as observation. When it enforces Leopold’s maneuver, all midwives explained the state of the fetus to the pregnant women. 98%( n=103) of midwives talked to a fetus while touching the abdomen of the pregnant woman. Midwives were explaining having noticed which touches to the pregnant women. At that time, they practiced “hear awareness of the pregnant-women oneself”, “promote awareness of the pregnant-women”, “ask the everyday life of the pregnant-women”, “talk about the preventive and the measures”, “talk to a fetus“, “talk about the situation”.

Conclusion
In the pregnant-women medical examination in the maternity home, Japanese midwives touched the pregnant women with a purpose while taking time. Midwives shared that they touched and made a judgment (diagnosis) with the pregnant women. In addition, midwives respected not only the pregnant women but also the personality of the fetus. It is considered that midwives aim at going to the delivery with the pregnant women in a good state mentally and physically through building a relationship with them by touching.
